Atelopus eusebiodiazi Venegas, Catenazzi, Siu-Ting, and Carrillo, 2008, Salamandra, 44: 164. Holotype: MUSM 7138, by original designation. Type locality: "Huamba, 22 km ESE (airline) of Ayabaca (4° 43′ 7.2″ S, 79° 31′ 50.88″ W) at an elevation 2,950 m, near Cerro Mayordomo, Provincia de Ayabaca, Departamento de Piura, Peru".

Distribution
Known from the type locality in the northern portion of the Cordillera de Huancabamba, Departamento de Piura, northwestern Peru, 2950 m elevation; also reported (see comment) from Parque Nacional Yacuri, Loja, Ecuador, 3368–3395 m elevation.

Three specimens (QCAZ 63506, 64462, and 69025) reported in Anfíbios del Ecuador (https://bioweb.bio/faunaweb/amphibiaweb/QCAZ/Especimenes/Especie/Atelopus%20eusebiodiazi) from the Parque Nacional Yacuri, Loja, Ecuador.
