Along forested streams on the eastern flank of the Cordillera Central, Antioquia, Tolima, and Caldas departments, Colombia, ca. 1100–1850 m elevation.

Comment
Unassigned to subgenus according to the original publication. Lynch, 2004, in Rueda-Almonacid et al. (eds.), Libro Rojo Anf. Colombia: 322-324, provided an account and map. In the subgenus Pristimantis, but unnassigned to species group by Hedges, Duellman, and Heinicke, 2008, Zootaxa, 1737: 118. Not assignable to a species group according to Padial, Grant, and Frost, 2014, Zootaxa, 3825: 127. See photograph, map, description of geographic range and habitat, and conservation status (as Eleutherodactylus fallax) in Stuart, Hoffmann, Chanson, Cox, Berridge, Ramani, and Young, 2008, Threatened Amph. World: 337. Ovalle-Pacheco, Camacho-Rozo, and Arroyo, 2019, Check List, 15: 392, reported the species (as Pristimantis aff. fallax) from Boyacá, Colombia, and commented on the range in Colombia.
