Cloud forest at 1800–2650 m in the municipalities of Pensilvania and Samaná, Department of Caldas, and municipality of Guatepé, Antioquia, on the eastern flank of the Cordillera Central, Colombia.

Likely related to Eleutherodactylus uranobates according to the original publication.
