Mayor San Marcos, 33: 1–7. MUSM 3743 designated lectotype by Wiens, 1993, Occas. Pap. Mus. Nat. Hist. Univ. Kansas, 162: 50; this action considered invalid by Lavilla, 1997, Cuad. Herpetol., 11: 75–80 (inasmuch as the holotype is lost, a "lectotype" designation is invalid). Type locality: "Huamachuco", La Libertad, Peru.

Distribution

Cloud forest to wet páramo habitats in the Cordillera Occidental of northern Peru (departments of La Libertad and Cajamarca), 2000–3520 m elevation.
