Type locality: "30 km southwest of Zapatero (about 10 km NE San José de Sisa (ca. 06° 45′ S, 76° 33′ W), 500 m, Provincia de Lamas, Departamento de San Martín, Peru".

Distribution

Known only from the type locality at the eastern base of the northern part of the Cordillera Central (10 km northeast of San José de Sisa, Provincia de Lamas, Departamento de San Martín, Peru) in humid tropical forest.
